How do you shift from problem-oriented to goal-oriented care in primary care settings? This project follows four networks on their journey to meaningful change.

In short
This research project supports and follows four learning networks that aim to implement goal-oriented care in primary care. Using a participatory and appreciative research approach, all stakeholders – including care providers, coaches and researchers – collaborate and co-create in their own working environment.

Need and relevance
The number of people with complex and chronic health needs is increasing globally. A holistic and inclusive care system is urgently needed. Although Flanders aligns with World Health Organisation recommendations, the shift from problem-oriented to goal-oriented care is difficult to realise in daily practice.

From approach to impact
The study applies participatory observation in natural settings. Researchers take on an engaged and flexible role, registering key insights on success factors and barriers. In co-creation with stakeholders, the project develops a practical guidebook for other organisations aiming to implement goal-oriented care. The project contributes to sustainable innovation and knowledge sharing in the healthcare field.
