How can people with chronic conditions regain control of their daily lives at home? Reablement offers a powerful answer in Flemish primary care.

In short
This project develops a culturally adapted reablement prototype for Flemish primary care. Two Reable-Labs will test this person-centred home intervention for people with chronic conditions receiving home care.

Need and relevance
An increasing number of people live longer with multiple chronic conditions and rely on home care. This requires a strong primary care system that promotes autonomy and quality of life. Reablement responds to this need by offering short-term, intensive support to help people regain essential daily and social activities. In Flanders, this concept has yet to be fully implemented.

From approach to impact
Building on previous research on the Flemish reablement context, this project co-creates a practical protocol and guidance with care professionals and people with lived experience. The intervention will be tested in two home care organisations. The findings will provide insight into how reablement can be effectively implemented in Flanders and will lay the foundation for future research on its impact in primary care.
